2026 CPT® Code Updates for Spine Surgery

The 2026 CPT^® code set has been updated with new codes and guidance for Spine Procedures.

Sacroiliac Joint Devices and Procedures: Revised Code Descriptor and New Guideline

27278 Arthrodesis, sacroiliac joint, percutaneous or minimally invasive, with image guidance, includes obtaining bone graft when performed, unilateral; placement of intra-articular device(s), without cortical piercing

2026 Physician Fee Schedule Value:

27279 Arthrodesis, sacroiliac joint, percutaneous or minimally invasive, with image guidance, includes obtaining bone graft when performed, unilateral; placement of trans-articular device(s) and/or intra-articular device(s) piercing the lateral or medial cortices of the ilium and the lateral cortex of the sacrum

2026 Physician Fee Schedule Value:

Percutaneous sacroiliac (SI) joint fusion can be achieved utilizing intra-articular or transarticular device(s). Code 27278 describes the placement of intra-articular devices that do not pierce the cortices of the ilium or sacrum. Code 27279 describes the placement of intra-articular and/or transarticular device(s) that pierce the lateral or medial cortices of the ilium and the lateral cortex of the sacrum. Use 27279 to report hybrid SI joint fusion that consists of placement of both intra-articular and transarticular devices, at least one of which pierces the cortices.

Annular Closure

New code +63032 has been established as an add-on code to CPT 63030 for bone-anchored annular closure device (e.g., Barricaid) implantation after lumbar discectomy for large annular defects.

63030 Laminotomy (hemilaminectomy), with decompression of nerve root(s), including partial facetectomy, foraminotomy and/or excision of herniated intervertebral disc; 1 interspace, lumbar

• +63032     with repair of annular defect by implantation of bone-anchored annular closure device, including all imaging guidance, 1 interspace, lumbar (List separately in addition to code for primary procedure)
